Your name, date of birth, contact number, email address, and home address. We use this to identify you, schedule your visits, and stay in touch about your care.
Sensitive health information
Your dental and medical history, treatment records, diagnostic images, X-rays, and clinical notes. Under the Data Privacy Act, this is sensitive personal information. We collect it only to assess, plan, and deliver your treatment safely.
Information from our website
When you submit the consultation form on drdsdental.clinic, we collect the details you provide: your full name, contact number, email address, preferred branch, and the concern or notes you share. We use this only to respond to your inquiry and arrange your appointment.
The form also includes an optional choice to receive dental tips, updates, and offers from Dr. DS. This is entirely your decision; you can opt out at any time, and leaving it unchecked has no effect on your care.
Cookies and analytics
Our website does not currently use cookies or analytics tools to track your activity. We may introduce these in future to understand how visitors use our site. If we do, we will update this policy and explain what is collected before any tracking begins.
How we use your information
We use your information to: provide and plan your dental treatment; confirm and remind you of appointments; respond to your questions; keep accurate clinical records; and meet our legal and regulatory obligations as a healthcare provider.
